Clinical History

75 year-old female patient, presented to emergency department with fatigue, confusion, and fever. History of primary billiary cirrhosis.

Clinical Discussion

Intravascular large B cell lymphoma is a type of B cell lymphoma characterized by proliferation of malignant B cells within lumina of small- and medium-sized vessels without parenchymal involvement. This is typically due to loss of cell surface molecules critical to transvascular migration such as CD29 and CD54. The skin and CNS are the most commonly affected organs. Microscopically, malignant lymphocytes expressing B cell markers are seen exclusively within vessel lumens. The disease usually progresses rapidly and prognosis is poor.
