Clinical History

69-year-old woman with speech difficulties. Neuroimaging showed edema in the left frontoparietal lobe and an enhancing nodule in the right occipital lobe.

Clinical Discussion

Intravascular large B cell lymphoma is a type of B cell lymphoma characterized by proliferation of malignant B cells within lumina of small- and medium-sized vessels without parenchymal involvement. This is typically due to loss of cell surface molecules critical to transvascular migration such as CD29 and CD54. The skin and CNS are the most commonly affected organs. Microscopically, malignant lymphocytes expressing B-cell markers are seen exclusively within vessel lumens. The disease usually progresses rapidly and prognosis is poor.
