Clinical History

80-year old male, with history of Nonalcoholic Steatohepatitis (NASH) cirrhosis and prior radio-frequency ablation (RFA) of segment 6 lesion that was a combined hepatocellular-cholangiocarcinoma. Now with new lesion on contrast CT in segment 5; the biopsy was performed from segment 5 lesion biopsy.

Clinical Discussion

Hepatocelluar carcinoma (HCC) is the most common primary malignancy of the liver. The main risk factors for HCC worldwide are viral hepatitis and cirrhosis. Microscopically, hepatocellular carcinoma has a trabecular pattern of growth with tumour plates that are larger than 4 cells thick and surrounded by a layer of flattened endothelial cells. This pattern, along with the pseudoglandular pattern (acinar growth pattern with proteinaceous material or bile within the lumina) are seen on the accompanying slide. Other growth patterns include clear cell, giant cell sarcomatoid, solid and pelioid (vascular lakes). Moderately differentiated HCC differs from well differentiated in that the tumour cells are larger with more distinct nucleoli.
