Clinical History
30 year old female, with one month history of cervical lymphadenopathy.
Clinical Discussion
Kikuchi-Fujimoto disease, also known as histiocytic necrotizing lymphadenitis, is an inflammatory process of unknown etiology. It commonly affects young patients (< 40 years old) of Asian descent. Patients typically present with fever and cervical lymphadenopathy which can persist for months but tends to resolve spontaneously. Microscopically, it is characterized by paracortical expansion by foci of eosinophilic necrosis with abundant karyorrhectic debris and numerous histiocytes (see CD68 stain). Histiocytes are characteristically elongated or crescentic in shape. In addition, plasmacytoid dendritic cells are found and tend to cluster, especially at the margins of the necrosis (see CD123 stain). Large transformed immunoblasts are found admixed within (see CD30 stain). Neutrophils and eosinophils are absent. Plasma cells are scarce or absent. In this biopsy, the compartmentalization of B- and T-cells remains with B-cells, localized predominately in follicles (see CD20 stain) and T-cells predominately in the paracortical region (see CD3 stain).
