Clinical Discussion

Common blue nevus is a benign proliferation of spindle-cell melanocytes most frequently found in the head and neck, extremities (dorsum of hand and feet), and buttock areas. Clinically, it presents as a well circumscribed blue or dark blue-black papule (< 1 cm), and is typically flat or slightly raised. Morphologically, it is composed of well circumscribed proliferation of spindle and/or dendritic-shaped melanocytes deep in the dermis between collagen bundles. It has no epidermal component, and is frequently admixed with melanophages in a sclerotic background in the dermis, minimal nuclear atypia, rare to absent mitoses and no necrosis. Prognosis is excellent, with very low malignant potential.
