Clinical History

Small bowel intussusception. Three large polyps found in the jejunum and ileum at laparascopy.

Clinical Discussion

Diagnostic features of Peutz-Jeghers polyp include the “arborizing” architecture with prominent smooth muscle comprising the “branches.” The presence of three P-J polyps suggests that this patient is highly likely to have Peutz-Jeghers syndrome and is at increased risk for malignancy. Dysplasia and carcinoma occur in 15-30% of patients and may arise anywhere in the GI tract, not necessarily confined to large P-J polyps.
